Fibre format indexing does not start with 0

Extended Description

The 'old' (stdlib rev 1119 and older) implementation of FibreIO in the stdlib enforced index ranges to start from 0, but the format was not symmetric. A later improvement made the format symmetric but indexes are now allowed to start from values greater than 0 (rev 1120). This is not in sync with SAC's indexing  and thus the FibreIO implementation should be updated again to enforce the same indexing rules.
